What to bring at your first visit....

Our patients’ privacy, security and care is of utmost importance. To minimize your wait we ask you to please come prepared.

Be sure to bring your photo id, insurance card, list of medications, list of surgeries and any other pertinent health information. For patients under the age of 18 we ask for the parent/guardian photo id. If you have legal guardianship or power of attorney for the patient receiving care please bring a copy of the paperwork.

No insurance card? No problem! The following information is a MUST in order to submit to your insurance:

Policy holders First and Last Name and Date of Birth
Insurance Company Name
Claims mailing address (for providers)
Insurance Phone Number (for providers)
ID
Group Number
